Insurance and Accident FinancialFunds Aid for Universitas Airlangga Students

Accident is an unfortunate incident that happens unexpectedly and unintentionally by way of force either physically or chemically .he cause should be present when the accident occurs, whether caused by a disease or not, and the direct consequence of the insured object, resulting in physical injuries, permanent disabilities or death suffered by one should be in accordance with the scope of the coverage.UNAIR coversall these.
The insurance coverage of students of 2013 is guaranteed by PT. Asuransi Umum Bumiputera Muda (Bumida) 1967, based on the MOUs on Accident Insurance for Universitas Airlangga Students Number 10850/H3/KU/2012 andNumber 109/PKS/BPM/IX/2012 dated September 5, 2012.

a. Students who experience accidents may apply to receive accident insurance to the Faculty by providing the following:
1. Letter of Report from the police
2. Proof of payment of hospital costs
3. If the student (the insured) suffers from a permanent disability,  a disability statement from a doctor indicating the percentage of permanent disability is required.
4. Copy of Driver’s Licence
5. Copy of Student Identity Card and National Identity Card /other valid identifications ,
6. If the student (the Insured) dies, a Death Certificate and photocopy of Marriage Certificate/or Family Certificate/or Legal Card of the legitimate beneficiaries is required

The Faculty proceeds student’s application to Vice Rector I and sends the copy to the Director of Student Affairs.
The student is required to fill out the LK-1 form of Bumida Insurance. The form should be signed by the doctor and stamped by the hospital. The form is to be signed by the Director of Student Affairs. The form can be obtained at the Welfare Unit of Directorate of Student Affairs.
For students of 2014 and afterwards, insurance claims are in the form of Accident Financial Aid for Universitas Airlangga Students:
Accident financial aid for Universitas Airlangga students is provided by Universitas Airlangga based on: Rector’s Decree No. 684/UN3/2016 dated March21, 2016. The aid is intended for students who experience an accident that results in death or permanent disability. The claim procedure set by UNAIR is as follows:
